Facilities Use Request Form

Please note that submitting this form does not guarantee you a reservation. After we receive your information we will email you to confirm whether or not we can accommodate your request.

Please follow these steps to request one of our facilities:

1) Save the MWR Facilities Use & Sponsorship Request Form to your computer:

2) Open the file on your computer, fill it out, save it, and attach it here:

3) Attach a picture of your Military ID here:

Per DOD policy 1000.13 we only review digital information to verify eligibility and never make copies.

4) Acknowledge the policies, sign the form and click submit below

5) We call you for your credit card number (if applicable)

6) We email you to confirm your reservation

All adults must have valid ID and all drivers must show valid vehicle registration and proof of insurance. If your ID states 'Federal Limits Apply' you will only be able to access the base as a sponsored guest and must show a second, federally recognized ID. If you are sponsoring a foreign national please provide 30 days notice. The policy below has examples of acceptable ID's and additional information. You are responsible for getting yourself and your party through base security. If you have specific needs or questions email us at MWR@uscg.mil.

I will follow all policies of TRACEN Petaluma and acknowledge that only beer and wine are permitted and that illegal drugs are a violation of Federal law and UCMJ. I understand my group will be asked to leave if in violation.
