Registering your child for an MWR Youth & Teen Program activity is a 3 step process:

1) Submit this form

2) The ticket office calls you for payment as needed

3) The Youth & Teen Program staff email you to confirm your child's enrollment

If this is your first time submitting your child's information on this page please fill out every applicable field below. Your submission will be stored securely on the Coast Guard network for future reference. If you already have your child's information on file, please only enter the applicable details such as which child you are signing up and for which event. File upload boxes are below for your convenience. 

 

This form will be sent directly to, and securely stored within, our MWR@uscg.mil email. This email is secure as is this website. Per DOD Policy 1000.13 we only review digital information to verify eligibility and never make copies. We systematically delete submissions from the Coast Guard network every 3 years. Upon your request, we will submit a General Data Protection Regulation (GDPR) inquiry to scrub your information from our website server. Our Youth & Teen Program is for MWR eligible patrons and their guests. Medical Information

I consent to MWR using audio/video/photos of my children for media purposes
I consent to my child(ren) participating in walks and field trips acrossTRACEN Petaluma
If signing up a teen, I consent to them participating in field trips and events that may be across TRACEN Petaluma and off-base
If signing up a teen, I give permission for them to play “M” rated video games

I hereby absolve USCG TRACEN Petaluma MWR, its agents and chaperones from any and all liability from any injury which may be suffered from participation in any MWR activity or event. In the event emergency treatment is needed and a parent/guardian cannot be reached, I authorize MWR and/or its agents and chaperones to take appropriate action as deemed necessary.

Uploading a picture of the front of your military ID so we can verify your eligibility is mandatory if this is your first time submitting this form. Attach any additional documents as needed such as medical records, doctors info etc. in image or .pdf formats.
